Transition Age Clinic

The Transition Clinic provides diagnostic evaluations, reevaluations, and/or mental health consultations related to autism or developmental disability. During the assessment, the clinic assesses strengths, skills and needs of youth. Both the individuals and family discuss their concerns with a family/person focused interdisciplinary team.

The interdisciplinary evaluation team consists of faculty and trainees from psychology and social work with other disciplines included as indicated. The evaluation process typically consists of 3 visits. Prior to the initial visit, the team reviews previous medical, educational and therapeutic records. During the initial clinic visit, a clinical interview is completed, and the assessment of the individual’s current abilities is initiated.  Assessments are completed by the interdisciplinary team and may consist of structured and unstructured observation, clinical interviews, and standardized testing. In week 2, the individual and their family return to complete the assessment.

During the third follow-up appointment, results and recommendations are discussed with the individual and family to develop supports and services. At the conclusion of the evaluation, individuals and parents or guardians are provided a behavioral profile of the individual, recommendations and/or relevant diagnoses. A formal report is provided to the individual and parents with a copy sent to the referring professional and/or primary care physician. All major insurance programs including Medicaid are accepted.
