Become empowered! Get connected with your school, your community, your world!

"Jake’s determination to make an impact in this world began when he was only hours old. Having beaten the odds of survival, he proved to those caring for him that he was meant to have a place in this world, a place that would demonstrate the meaning of courage, hope, and purpose. With the guidance of his family, mentor and others, Jake has become a young man who has the skill and confidence to reach dreams never thought to be possible."

What is the E-Mentoring Program?
The E-Mentoring Program uses technology and mentoring to prepare a student with disabilities for their transition from high school to employment or college. Each student is supported by a mentor, who helps them navigate a self-directed online course that results in the development of a personalized Transition Portfolio.
